What is NDIS support coordination?

NDIS support coordination is a funded support that helps you understand your NDIS plan, choose and connect with providers, and coordinate your supports so you can work towards your goals. It’s usually included in the Capacity Building part of your plan when the NDIA decides you need help to put your plan into action. A support coordinator works for you, helping you make your own choices.

How a support coordinator can help

Your support coordinator can:

Explain your plan — what each budget is for, how your plan is managed and what you can and can’t use funding for.

Help with connecting with NDIS providers — finding options that suit your goals, location and preferences, and setting up service agreements.

Coordinate your supports — making sure your providers, therapists and support workers work together.

Build your skills — so you can manage more of your plan yourself over time.

Help with problems — when a service isn’t working, needs change or a crisis comes up.

Prepare for plan reassessment — gathering progress information and reports so your next plan reflects your needs.

Link with other systems — such as health, mental health, housing and education.

The three levels of support coordination

Level What it involves Who it usually suits
Level 1: Support connection Building your ability to connect with informal, community and funded supports, and to understand and use your plan People who need some help getting started and expect to manage on their own soon
Level 2: Coordination of supports Helping you understand your plan, connect with providers, coordinate supports and build your capacity to manage them Most participants who have support coordination
Level 3: Specialist support coordination Higher-level support from someone with specialist qualifications, for situations with high risk or complexity People with complex needs, such as housing instability, involvement with the justice system, or several systems interacting at once

Our commitment to your choice

Care Expert Australia provides support coordination and other NDIS supports. We know this can create a conflict of interest, so we manage it openly:

  • You choose. We’ll always present a range of providers, not just our own services, and you’re free to choose anyone.
  • No pressure. Using our support coordination doesn’t mean you have to use our other services, and the reverse is also true.
  • Transparency. We’ll tell you whenever an option involves Care Expert’s own services.

If you’d prefer a coordinator who doesn’t provide any other services, we’ll respect that and can point you towards independent options.

Support coordination is changing: what it means for you

The Australian Government has announced a new commissioned Support Coordination and Connection service, planned to start from 1 July 2028. Until then, support coordination continues under the current arrangements. Details of the new model are still being designed. We’ll keep our participants informed as things change, and help you through any transition.

Who may benefit from support coordination

Support coordination may suit you if you:

  • have a new NDIS plan and aren’t sure where to start
  • have several providers or complex supports to coordinate
  • are going through a big change, such as moving home, leaving school or leaving hospital
  • are exploring housing options like SIL or SDA
  • are dealing with more than one system, such as health, mental health or justice

Is support coordination covered by your NDIS plan?

Support coordination is funded under Capacity Building supports when the NDIA includes it in your plan. If it isn’t in your plan and you think you need it, you can raise this with your Local Area Coordinator or planner at your next plan reassessment, or ask for a plan change if your situation has changed. For more information, see the NDIS guide on support coordination.

Frequently asked questions

What does an NDIS support coordinator do?

An NDIS support coordinator helps you understand your plan, choose and connect with providers, coordinate your supports and build your ability to manage your plan. They also help when problems come up and when you prepare for plan reassessment.

What are the three levels of support coordination?

The three levels are Support Connection (Level 1), Coordination of Supports (Level 2) and Specialist Support Coordination (Level 3). The NDIA decides which level, if any, is included in your plan.

How do I get support coordination in my NDIS plan?

Talk to your Local Area Coordinator or NDIA planner about why you need help to use your plan. It’s usually considered at planning or reassessment, or when your circumstances change.

Can I change my support coordinator?

Yes. You can choose a different support coordinator at any time, subject to the notice period in your service agreement.

Can my support coordinator also provide my other supports?

Yes, but the provider must manage the conflict of interest. You should always be offered a choice of providers and never be pressured to use the coordinator’s own services.

Is support coordination changing?

Yes. The Government plans a new commissioned Support Coordination and Connection service from 1 July 2028. Until then, support coordination continues under current arrangements.

What's the difference between a support coordinator and a plan manager?

A support coordinator helps you use your plan and coordinate providers. A plan manager handles the financial side, such as paying invoices and tracking budgets. You can have both.

Do you provide support coordination in Bayswater?

Yes. We’re based at 3/15 Railway Parade, Bayswater, and support participants across. Call 03 9798 8893 to talk with a coordinator.
